Connect Fort Bend has been established to sustain the well-being and vibrancy of communities in Fort Bend County. Through community collaboratives, Connect Fort Bend provides a wide range of social services for Fort Bend residents to reach their full potential. The focus is to ensure children/youth develop skills and abilities and have supports to thrive during their developmental years, contribute positively to society, and become healthy adults.
WE FOCUS ON POSITIVE YOUTH DEVELOPMENT
- Youth are engaged in productive and constructive activities to provide them essential skills for post-secondary success
- Youth are building upon their strengths and developing the skills, competencies, and values they need to contribute positively to society
- Youth have the support they need to develop academic, social and emotional, life, and college and career readiness skills for now and their adult life.


We engage youth in hands-on 4-H projects in areas like health and wellness, STEM, agriculture, and civic engagement in a positive environment where they receive guidance from adult mentors. Our program 1) empower youth to become leaders today and throughout their lifetime, 2) allow youth to explore various careers and career pathways, 3) empower youth to make decisions for healthy lifestyles and 4) allow youth voice – giving them an opportunity to express who they are and how they can make their lives and community better. Most importantly our program helps young people and their families gain skills to be proactive forces in their communities and develop ideas for a more innovative economy.
Services
Services are aimed to support the healthy development of youth, sustain great communities, support schools, and keep Fort Bend a viable community to live. Programs, projects, and initiatives are coordinated to improve the health, well-being, and quality of life of Fort Bend residents – addressing the social determinants of health.

Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.
